Abstract: Cardiac rehabilitation (CR) programs represent a cornerstone in the management of heart failure (HF) offering structured, multidisciplinary interventions designed to improve physical psychological, and social outcomes for patients The study evaluates the effectiveness of CR programs through a hands-on practical approach conducted at my provide clinic and Al Zahra teaching Hospital Over a six-month period, 100 HF patients participated in a 12-week CR program that included supervised exercise training nutritional counseling psychological support and educational workshops Data were collected at baseline mid-program (6 weeks), and post-program (12 weeks) to assess clinical outcomes, adherence rates, quality-of-life improvements and psychological well-being. The findings demonstrate significant improvements in functional capacity, as evidenced by a 28% increase in the 6-minute walk test enhanced mental health with a 50% reduction in anxiety and depression scores and reduced hospital readmissions. Despite these positive outcomes, challenges such as low adherence rates, logistical barriers and motivational deficits were identified as key obstacles to program success. These challenges highlight the need for innovative strategies, such as telehealth-based interventions and personalized motivational counseling, to enhance participation and long-term engagement This study underscores the transformative potential of CR programs while emphasizing the importance of addressing real-world implementation barriers to maximize their impact on patient outcomes.
