A prospective research at a tertiary care hospital examining the aetiology of hypokalaemic periodic paralysis

Abstract: Introduction and Background: A rare neuromuscular condition known as hypokalaemic periodic paralysis (HypoKPP) causes paralysis or weakness of the muscles at specific intervals and is linked to low serum potassium levels. Also stressed was the significance of prompt diagnosis and treatment in avoiding handicap in the long run.
Materials and Methods: A tertiary care institution oversaw the 12-month prospective observational research. 50 patients were included in the study; their clinical presentation, low serum potassium levels, and the exclusion of other possible causes of weakness were the criteria for hypoKPP suspicion or confirmation. This study was conducted at the Department of General Medicine, Madha Medical College, & Research Institute, Kundrathur Road, Kovur, Chennai, Tamil Nadu, India from February 2021 to January 2022. Electromyography (EMG), thyroid function testing, genetic screening for familial HypoKPP mutations, and a thorough medical history.
Results: 50 patients were examined, with 36 (72%) found to have hereditary HypoKPP and 14 (28%) identified with acquired variants of the condition. There was a 60% male preponderance, and the average age of onset was 22.5 years. Exercise (42%), high-carbohydrate meals (36%), and stress (22%), in that order, were the most common causes of hypokalaemic episodes. Episodic serum potassium levels were 2.1 mmol/L on average, although they were as high as 3.0 mmol/L.
Conclusion: The majority of patients in this group have familial hypoKPP, which is caused by mutations in the CACNA1S and SCN4A genes. The findings of this study highlight the value of genetic testing for the diagnosis of HypoKPP and the customisation of treatment plans to maximise patient well-being.
