To study Latent TB prevalence among patients with rheumatoid arthritis and ankylosing spondylitis

Abstract: Introduction: The possibility of a relapse of tuberculosis infection in those receiving tumor necrosis treatment for rheumatic musculoskeletal disorders. The study's goals were to conduct a cross-sectional investigation on the relationship between diabetic pulmonary tuberculosis and clinical and radiological symptoms in people with diabetes mellitus.
Materials and methods: Patients who were being evaluated for bDMARD therapy were included in the trial. The "4S symptom complex (current cough, fever, weight loss, and night sweats) for TB" screening approach was used in conjunction with an enhanced Mantoux test that included 10 tuberculin From March 2019 to February 2020, this study was conducted in the pulmonology department of the RVS Institute of Medical Sciences in Chittoor, Andhra Pradesh.
Results: A total of 730 individuals were evaluated for bDMARDs: 265 with rheumatoid arthritis, 400 with axial spondyloarthritis [axSpA], 34 with psoriatic arthritis, and 31 with other conditions. A total of 267 (36.6%) people tested positive for LTBI. For six months, they received isoniazid monotherapy; alternatively, they received four months of rifampicin + isoniazid therapy. Chemoprophylaxis was initiated one month after bDMARDs were started. During the follow-up, five (0.68%) of the patients experienced active TB illness. Out of 2930 "control" patients who had the same conditions but had never received bDMARDs, 18 (0.61%) had active tuberculosis. There was no difference in the percentage of patients who experienced active TBI during the same follow-up period between those who received bDMARD treatment and those who did not. Not a single research subject displayed "4S" symptoms.
Conclusion: Patients with active TBI were successfully identified with the use of the "4S complex," standard chest radiograph, and augmented Mantoux testing in conjunction with the QFTG test for LTBI screening.
